Unika Medical Centre is fully integrated with the Ontario eServices Program. To submit a referral:

1. Log in to your EMR’s e-Referral module (e.g., Ocean/Accuro/PS Suite/EMR-integrated OTN e-Referral).
2. Search for “Unika Medical Centre” or “Dr. Michael Gofeld” in the e-Referral directory.
3. Select the appropriate service category (Pain Management / Interventional Pain / MSK & Spine / Sports).
4. Attach relevant documentation: imaging reports, consultation notes, medication history, and any previous interventions.
5. Submit the e-referral. You will receive automated updates through the eReferral system regarding triage status and appointment booking.
